USPS Form 1583 is an official authorization form that allows a Commercial Mail Receiving Agency (CMRA), such as Florence Mailbox & Business Center, to accept mail for individuals or businesses.

This form verifies:

  • Your identity

  • Your physical address

  • Your authorization for us to receive mail on your behalf

Required Identification

When submitting Form 1583, customers must provide:

  • Two valid forms of identification

    • One photo ID (such as a driver’s license or passport)

    • One secondary ID (such as a vehicle registration, lease agreement, or utility bill)

All identification must be current and legible.

Completing Form 1583

In Person

Customers may complete and submit USPS Form 1583 in person at our location. Our staff will assist you through the process and verify your identification on-site, making notarization unnecessary when completed in person.

USPS Form 1583 Notarization Checklist

Completing Form 1583 Online (Notarization Required)

If you choose to complete USPS Form 1583 online or remotely, the form must be notarized before submission.

Because USPS requires identity verification, online forms cannot be accepted unless:

  • The form is fully completed

  • The form is signed in the presence of a licensed notary public

  • The notary section is properly completed and stamped

Once notarized, the completed form and copies of your required identification may be submitted to Florence Mailbox & Business Center as instructed.

Forms that are not notarized cannot be accepted.
